Integrative Primary Care
Our integrative primary care appointment is designed to support your family’s ongoing health with well-child checks, annual physicals, and basic bloodwork. If additional or specialized testing is needed, we offer those through a Functional Medicine visit. Direct messaging with providers isn’t included with this appointment; a membership is required for secure provider messaging. Each 50-minute visit is $250, giving you personalized care and guidance for your family’s well-being.
All non-member appointments require a nonrefundable $75 scheduling fee, which will be applied toward the cost of your first appointment.
Functional Medicine
This Functional Medicine appointment provides a thorough, root-cause evaluation, including a full assessment and physical, for patients ready to dive deeper into their health. Recommended testing may include GI-MAP, DUTCH, ZRT panels, Myco-Metabolic, and Organic Acids to uncover underlying contributors and guide your personalized care. The package includes an initial 90-minute visit and a follow-up within 30 days to create a plan for healing. The cost for both visits is $500, with any testing fees paid separately. Please note that direct messaging with providers isn’t included with this appointment; a membership is required to access secure provider messaging. Additional follow up visits are $250 for 50 minutes.

MEMBERSHIP
Our membership provides personalized, comprehensive integrative primary care. Members enjoy direct access to their provider in the office, by phone, or virtually, longer and unlimited visits, and one-on-one relationships that allow us to spend more time understanding and supporting your health. You can schedule in-person or telehealth visits, and we prioritize same-day or next-day sick appointments whenever possible. Membership also includes access to your electronic health records, comprehensive care coordination with testing and specialist referrals, a yearly physical exam, and an optional discounted Functional Medicine visit for a deeper, root-cause evaluation for $250, with follow-up care included.
Pricing is simple and transparent: individual memberships are $125 per month, couples $200 per month, and families up to four members $275 per month, with each additional child up to age 24 at $20 per month, and a 10% discount for annual payments. Children 25 and up will require their own individual membership.
All initial new membership visits require a nonrefundable $75 scheduling fee, applied to the first appointment, with families scheduling multiple member appointments charged only once. Our membership model makes holistic, functional healthcare accessible, convenient, and focused on you and your family.
