Policies & Patient Information
At Functional Now, we want you to feel informed and confident about your care. The policies below outline important information about our practice, including communication, scheduling, payments, cancellations, and other aspects of your care.
Please take a moment to review these policies before your visit. If you have questions about any of them, we’re always happy to help.
Scheduling & Appointments
Appointments are scheduled based on provider availability and the type of care being provided. Members receive priority scheduling as part of their membership benefits.
To help ensure appointment availability and minimize no-shows, a nonrefundable $75 scheduling fee is required for all non-member visits and initial new-member appointments. This fee is applied toward the cost of your visit.
Patients are responsible for arriving on time and being prepared for their scheduled appointment. Arriving late may reduce the amount of time available for your visit and, in some circumstances, may require the appointment to be rescheduled.
If you need to cancel or reschedule an appointment, please provide at least 24 hours' notice whenever possible.
Cancellations & No-Shows
To help us maintain appointment availability for all patients, appointments cancelled or rescheduled with less than 24 hours' notice, as well as missed appointments (no-shows), may incur a $50 fee.
We understand that unexpected circumstances happen. True emergencies, including significant illness or family emergencies, will be considered on a case-by-case basis, and the fee may be waived at the practice's discretion.
Repeated late cancellations or no-shows may result in a conversation with the patient about scheduling needs and, when applicable, a review of membership status. In some circumstances, repeated missed appointments may result in termination of the membership agreement.
Membership
Membership terms, fees, initiation fees, cancellation, and other membership requirements are outlined in the Membership Agreement provided before enrollment.
If a membership is cancelled and 60 days or more pass before the patient chooses to rejoin Functional Now, the $200 initiation fee will apply again.
Communication & Patient Portal
We value clear and timely communication with our patients.
Members may use the OnPatient portal for secure communication with the practice, as well as for accessing available health information and completing applicable forms.
Non-members should contact the office by phone or text for routine questions, prescription requests, and other communication. OnPatient access and messaging are available to members as part of the membership experience.
The patient portal and text messaging are intended for routine communication. Messages received after office hours will be addressed during the next business day.
For urgent or time-sensitive medical concerns, patients should contact the office directly or seek appropriate urgent or emergency medical care.
Prescription & Refill Requests
Member Prescription Policy
Members may request prescription refills through the OnPatient portal or by contacting the office.
For controlled substances, members must have an in-office appointment with the provider at least every six months for continued prescribing, or more frequently when clinically appropriate. Controlled-substance refills are provided when requested rather than automatically sent on a monthly schedule, as some medications may not be taken every day.
Patients should contact the office when they are running low on medication to request a refill. Refills are subject to provider review, clinical judgment, and applicable prescribing requirements.
For non-controlled medications, the provider may authorize a 6- to 12-month supply when clinically appropriate. The length of the prescription is determined by the provider based on the patient's individual circumstances.
Factors that may affect refill authorization include the need for updated laboratory testing, vital signs, medication monitoring, symptom follow-up, or an updated treatment plan.
Patients must remain current with any required follow-up appointments, laboratory testing, or monitoring in order to continue receiving prescriptions. Medication refills are not guaranteed and are always subject to provider review and clinical judgment.
Non-Member Prescription Policy
Non-members may contact the office by phone or text for prescription refill requests. OnPatient messaging is available to members only.
For controlled substances, patients must have an appointment with the provider at least every three months to continue receiving refills. Patients should contact the office when they are running low on medication to request a refill. Because controlled substances are not necessarily taken every day, refills will be provided when requested rather than automatically sent on a monthly schedule.
For non-controlled medications, the provider may authorize a 6- to 12-month supply when clinically appropriate. The length of the prescription is determined by the provider based on clinical judgment, including whether updated labs, vital signs, symptom monitoring, or other follow-up is needed.
Patients must have been seen by Functional Now within the previous 12 months to receive continued refills. It is the patient's responsibility to schedule any necessary follow-up appointment before running out of medication.
If a patient is completely out of medication and requires a refill, an appointment will be required. When clinically appropriate, the provider may provide a one-month supply with no refills. No further prescriptions will be provided until the required appointment has been completed.
All prescription and refill requests are subject to provider review, clinical judgment, and applicable prescribing requirements.
Telehealth
Initial visits are conducted in person. Telehealth and phone appointments are available for established patients following their initial visit.
Not every concern or type of appointment can be safely or appropriately managed through telehealth or by phone. The provider may determine that an in-person visit, examination, laboratory testing, or other evaluation is necessary.
If a telehealth connection cannot be established or is interrupted, the provider may contact the patient by phone or determine that the appointment needs to be rescheduled.
After-Hours & Urgent Medical Concerns
Functional Now does not provide emergency or continuous after-hours medical care.
For medical emergencies, patients should call 911 or go to the nearest emergency department.
For urgent concerns that cannot safely wait until the office is open, patients should seek appropriate urgent care or emergency medical evaluation. Please do not rely on portal messages, text messages, or voicemail for emergency or time-sensitive medical concerns.
If you receive care at an urgent care facility or emergency department, please notify your Functional Now provider afterward so that appropriate follow-up care can be coordinated when needed.
